ACTIONS REQUIRED BY ALL COACHES

Eastview Basketball Head Coaches and Assistant Coaches must complete the following tasks prior to engaging in any formal activities with their team:

1) Complete a criminal background check.  Background checks must be completed annually and must be submitted for each individual sport for tracking purposes (EVAA is not charged extra for checks performed on the same person for multiple sports).

2) Complete online concussion training and follow the instructions provided below, initially and once every 3 years as of September 1, 2011, in accordance with Minnesota State Statute

2) After completion of the training and quiz, save the PDF on your computer.

3) Email certificate to concussionsEVAA@gmail.com

4) If you coach multiple sports, be sure to state that in the email.  If in the future you begin coaching another sport please email concussionsEVAA@gmail.com to update your profile.
